Remove duplicate ncd_institution and ncd_hl7_producer rows

Description

The recent data load added a small number of duplicate ncd_institution and ncd_hl7_producer rows. Specifically there are a pair of ncd_institution rows with name 'Wishard' and 'WISHARD', and a pair with name 'St Francis ADT' and 'St France Adt'. The duplicate ncd_hl7_producer rows are for these duplicate ncd_institution rows.
