encounter datasource needs encounter_type key added

Description

The encounter datasource needs expanded to support other keys. Specifically we need the encounter_type key supported for the HIVPOSITIVE rule for AMPATH. The key encounterDatetime is supported right now. We should add any other keys that seem like they might be useful as well.
